PRIMARY CARE NETWORK

A collaborative community-based network of health care providers dedicated to improving patient health care experiences in our geographic area.

Team Based Care

Each Primary Care Network is a geographic region with a dedicated clinical team of allied health professionals to support the Family Doctors and Nurse Practitioners (NPs) within that region.

In the North Peace, this represents a partnership between the North Peace Division of Family Practice, Northern Health and our local First Nations and Indigenous partners. Simply put – it is everyone working together in teams to provide all the primary care services to a local community.

The purpose of the North Peace PCN is to work collaboratively with our partners towards a local, coordinated, and comprehensive primary care system that promotes and advances accessibility to a primary care team and high-quality care for all members of the community.

A Primary Care Network includes:
  • Enhancing patient care using a team-based approach to care.
  • Supporting each other and work to their own strengths.
  • Ensuring patients are linked to other parts of the system, including the health authority’s specialized community services programs for high risk and vulnerable population groups.
  • Collectively working to increase access and attachment to primary care.

North Peace PCN Key Focus Areas:

The newly established medical clinic at the Women’s Resource Centre’s The Healing Place

The program’s goal is to provide vital health and wellness services to low-income families and underserviced populations in the region. PCN Resources include a full time Mental Health Clinician, full time Life Skills Worker, full time Registered Nurse and support from the Primary Care Clinic Nurse Practitioner.

Engage with Indigenous Communities
Collaborate with and listen to Indigenous partners to understand priorities, and incorporate principles of respect, reconciliation, and cultural safety into any actions taken.
Enhancing team-based care in the Primary Care Homes

Our service plan includes adding Primary Care Nurses (LPNs) to be co-located in family physician clinics. In 2024, there were more than 5,000 patient visits provided by 3 LPN’s in these clinics which represents a valued contribution toward team-based care aimed at improving access to care for our patients and families.
